The City Requests Certificate of Insurance ("COI") from applicant and asks to be named additionally insured on a CG 20 26 – Additional Insured Designated Person or Organization form issued by your insurance company.
NOTE:
Inflatables (bounce houses, bull riding and other similar activities are not allowed by City unless insurance company providing the equipment names the City as additionally insured (CG 20 26). Add the following language to special event application:
I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS THE CITY OF FORT BRAGG, ITS EMPLOYEES, AGENTS, VOLUNTEERS, INDEPENDENT CONTRACTORS, OFFICERS, ASSIGNS AND SUCCESSORS (RELEASEES) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES OR DAMAGES, SUSTAINED BY ME AND/OR MY CHILD, CAUSED OR ALLEGED TO BE CAUSED, IN WHOLE OR IN PART, BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, AND FURTHER AGREE THAT IF, DESPITE THIS RELEASE, I, THE CHILD OR ANYONE ON THE CHILD’S BEHALF MAKES A CLAIM AGAINST ANY OF THE RELEASEES NAMED ABOVE, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEYS’ FEES, LOSS, LIABILITY OR DAMAGE, OR ANY OTHER COST THAT MAY OCCUR AS THE RESULT OF SUCH A CLAIM.
Bounce House/Inflatables
Will you be having a bounce house at your event?
City requires all parties with inflatable jump houses to provide proof of insurance from the Bounce House Company. The insurance should be a General Liability policy in the amount of no less than $1,000,000 per Occurrence and $2,000,000 General Aggregate. The policy must also list the City as additional insurance and the certificate holder as outlined in the Insurance Requirements listed above issued by Insurance Company. Waiver
